You are invited to submit Cookbooks and/or 'Culinary Arts' Entries into the Awards Program. You may enter yourself for an Award/s, or have someone nominate you. You may enter as many categories as you wish. However, please use a separate Entry form for each category entered. You can view the different submission requirements listed on the next page for each category. |

ENTRY FORMS:
Download and print out the three pages Entry Form/s from your computer, complete the sections required on pages two and three, and submit all three pages with your entry materials. Electronic submissions will not be accepted. A group entry for an Award requires one Entry form per person. Example: Food Stylist & Photographer. Clip the 'group' Entry forms together and submit with the requested materials. If multiple category submissions are sent in one envelope, make sure the Entry forms and materials requested are correctly categorized together .
ENTRY FEES:
The Entry Fees for all Cookbooks and Culinary Arts categories are:
US $65.00 per Single Entry* US $100.00 per Group Entry** US $130.00 per Triple Entry***
The Culinary Magazine requires two persons to enter. It is considered a group entry US $100.00
The 'Culinary Demonstrator' can be a Single Entry* - US$65.00, or a Group Entry** US$100.00. The Culinary Demonstration Group Entry can feature two people on the DVD or Video Tape; or the Group Entry can be from the Demonstrator & Photographer who made the DVD or Video Tape.
Checks or Bank Drafts in US Dollars can be made out to Cordon d'Or - Gold Ribbon Inc. The closing date for receipt of entries is September 30, 2008. Entries postmarked after September 30, 2008 will not be accepted. No exceptions permitted. The decision of the Panel of Judges is final.

Award Winners will be notified by January 31, 2009. The Awards Presentations will take place in Florida in 2009 (date to be announced). If the Winners are unable to attend, they can arrange for someone to accept the Award on their behalf. The Winners names will appear on the Cordon d'Or - Gold Ribbon Awards Website on January 31, 2009 |

Anyone who prefers to send in their Entry through UPS, FedEx or DHL may do so. However, they will NOT deliver to a P O Box. Please EMAIL cordondor@aol.com for street mailing address instructions. People who need W9 tax forms may also contact us by EMAIL and the form will be sent to you. Award Winners in the Cordon d’Or - Gold Ribbon Cookbooks and Culinary Arts Culinary Academy Awards 2008 Program, who are unable to attend the Culinary Academy Awards Event in St. Petersburg, Florida in 2009, can notify us. We use UPS to ship Awards, but we do not cover the costs for shipment. Winners will be notified about UPS costs, and we will arrange the shipment. |
